Permission to Ride with an Adult Leader

Do you give the adult youth leaders permission to pick-up/drop-off your teen for youth related activities/events this year?

This includes, but is not limited to indoor and outdoor games and activities that may incur injury (paintball, frisbee, tag, etc.), events like Pumpkin Chunkin, the State Fair, Paintball, and traveling to those events with a background checked Youth Leader

Release from Liability

I release the youth leaders, home owners (when an event it in house), and Cornerstone Church from any injury or medical event that arises from my teen's transportation and participation in youth group this year. By selecting "yes" I am adding my digital signature to this form.

Dear parents,

Our youth group meets on Wednesdays for free meals from 5:30-6:15pm, and what we call “Youth Group” from 6:15-7:45pm. On Wednesdays we’re focused on inviting friends to a fun evening that ends with God’s Word is opened and discussed. We’re currently going through “10 Questions Every Teen Should Ask and Answer about Christianity.”

Our youth group also meets on Sundays, before the worship service, at 9:30am in Pastor Joshua’s or Tim’s office. On Sundays we do a deeper dive into the topic we touched on Wednesday night.

Our leadership team cannot, and does not desire to, replace parents. As parents, we need to be growing spiritually if we are to expect the same of our teens.

There is no “youth pastor,” but we have two pastor-dads who love spending time with teens and seeing families grow spiritually.
